Home Health Registered Nurse (RN) | Dewey, Arizona
Experienced and compassionate Home Health Registered Nurses are needed for a 13-week contract position, providing skilled nursing care to patients in their homes. This opportunity calls for nurses who thrive in a field-based environment, can manage a flexible caseload, and deliver a high standard of patient-centered care.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
Key Responsibilities:
- Perform comprehensive assessments using the OASIS tool and develop individualized plans of care.
- Deliver direct skilled nursing interventions such as wound care, IV therapy, medication administration, and chronic disease management.
- Monitor patient progress, detect changes in condition, and communicate with interdisciplinary teams and physicians.
- Educate patients and families regarding disease management, treatment plans, medications, and health maintenance.
- Ensure timely and accurate electronic documentation using Home Care Home Base (HCHB).
- Coordinate care by collaborating with physical therapists, occupational therapists, social workers, and home health aides.
- Attend case conferences, staff meetings, and participate in ongoing education and training.
- Maintain compliance with all relevant agency protocols and regulatory standards.
Qualifications:
- Active and unrestricted RN license in the state of practice.
- Minimum one year of recent field-based Home Health RN experience.
- Demonstrated proficiency in Home Care Home Base (HCHB) and OASIS documentation.
- Excellent clinical assessment skills and ability to work independently.
- Reliable transportation, valid drivers license, and auto insurance.
- Strong organizational, communication, and time management abilities.
- BSN preferred; experience with caseloads of 2532 points per week and familiarity with Medicare/home health compliance are advantageous.
